Diabetes Management Center
The Diabetes Management Center at Terrebonne General Medical Center was
created to train, teach, develop, and improve the patient self-care
skills essential to diabetes management so that patients may live a
healthy life with diabetes.
The Diabetes Self-Management Education Program at TGMC has been recognized by
the American Diabetes Association for Quality Self-Management Education. This
program was the first facility of its kind in the Tri-Parish area to be so recognized.
The Center is coordinated by a certified diabetes RN educator with over 16 years
experience in teaching diabetes and a registered dietitian. In addition, TGMC is
actively involved in community outreach and advocacy programs concerning diabetes.
We provide a complete education/training experience based on the needs of the patient
and family, and the recommendations of the referring physician. Training programs are
provided in two formats: one-on-one individualized sessions or small group
sessions. Patients are encouraged to bring a family member.

Referral
Any physician may refer a patient to the Diabetes Management Center by
completing the order/consultation form.
